Key Point: As Accountable Care Organizations (ACOs) and bundled payments become more common; discharge location will become an important element in preventing readmissions and in designing an effective post-acute continuum of care.

Note that many inpatient reports provide service line and DRG level details. Many reports show details of the patient encounter (e.g. admit/discharge date, patient origin (zip code, city, etc.), length of stay, reimbursement, etc.), though no specific patient names or numbers are supplied.
